I myself do not make home visits. I rely on the Care Coordinators to make these visits. It has worked for us, I get reports from them after their visits. Also, I communicate weekly with the ones in the community.
We require at a minimum a Face -to Face in the home/residence every other month.
We require the same frequency for visits no matter of setting or individual’s needs. We utilize a safety home checklist on all visits.

We require the same visit except if they live with their family, we don’t use a scale.
We try to meet with the 24/7 clients every 3 months.
When they live with family, we visit some, but mostly do phone calls. Alot of those families don’t want us coming over alot. Or they have a really busy schedule and not home often.
We do require in person visits quarterly and at least 1 contact by phone or zoom each month for all members we serve
We require our coordinators to visit each assigned client at least once per week. We apply the same frequency across the board.
