We are wondering if facilities are required to submit incident reports to DDS for inclement weather closures.
There is a line in the requirements stating “interruption of services for 2 hours or more”.
The last time we submitted a “general” incident report for the facility closing, Summit and ATC replied back that they do not accept those because they are not member specific incident reports.
I’ve emailed Lee Honorable and asked for clarification but we also wanted to reach out and see if any of the DDPA members know if incident reports are required when inclement weather prevents the facility from opening.
- Yes, we are required to notify DDS and all PASSEs. If you are part of CACFP, you are also required to notify your CACFP representative with your agency ID. I usually copy everyone on one email so I don’t have to send multiple messages, and then attach the incident report.
I span the dates on my reports as needed. For example, if we missed all of last week, I sent one report on Friday covering that week. Then I completed another report for the following two days so I could finalize everything. I believe this is the most recent incident report. Due to this is unforeseen events.
- If the center is closed, then no.
- We do not submit reports for snow days
- We used to submit when our ADDT was closed for weather, but since going to the PASSE system we have not due to the very reason that this provider stated.
- I have submitted them to the central address only
