Our member use 4 different email address to send the 1910 form for ADDT/EIDT:

Care Coordinators submit incident reports without letting providers know
Daily update requests emails for the hospitalized client (Too excessive) (emails)

Death report within 1 hour
In reality, getting on a computer to make a report within an hour is the last thing on our mind when you are dealing with actual client death. Overall it is very chaotic and emotional. Taking care of the situation, client, other clients, and staff come first.
Here is 1 example that i personally encountered:
Client had a heart attack at ADDT in front of other clients:
Assign staff to follow ambulance with the client. Make sure to have client’s info.
Assign another staff to contact his family members.
Clients who witnessed were all upset so we had to take care of them.
Collect information from witnesses what happened
Call those clients family members to explain what happened
Communicate with DSPs for those clients what happened bc they need extra attention
Take care of upset staff
Schedule another staff who can go to hospital to release original staff
If we MUST report the death within one hour, can you give us 1-800 number or something that we can call and make a report instead of the portal?
