What are your agency’s current practices for in-person visits for CES Waiver services individuals? Specifically for those who live in the community, not those in residential facilities. Many years ago, the old standards required quarterly in person visits, but standards no longer require that. Do you require your waiver managers/coordinators to conduct regular in-person visits with individuals on their caseload? If so, what is the frequency of these visits? Do you apply the same visit frequency requirement for all individuals supported under the waiver program, or do you use an assessment or acuity scale to determine the appropriate frequency of in-person visits based on each individual’s needs? If you do use an assessment or acuity scale, would you be open to sharing the name of that tool with us.
I myself do not make home visits. I rely on the Care Coordinators to make these visits. It has worked for us, I get reports from them after their visits. Also, I communicate weekly with the ones in the community. Kathy Mullinix kmullinix@oectrs.org We require at a minimum a Face -to Face in the home/residence every other month. We require the same frequency for visits no matter of setting or individual’s needs. We utilize a safety home checklist on all visits. Yes, 1x week. We require the same visit except if they live with their family, we don’t use a scale. Kyauna Wells kwells@groupliving.org We try to meet with the 24/7 clients every 3 months. When they live with family, we visit some, but mostly do phone calls. Alot of those families don't want us coming over alot. Or they have a really busy schedule and not home often.
