Payee – Estate As a waiver provider that is the rep payee for the client, do you have a form for the client to sign stating if he/she is deceased who they want their remaining money and furniture to go to? ILS: We do not currently have a form for clients to designate where their remaining money and furniture should go in the event of their death. but, we would like to implement one. Bost: We do have a form in place for this purpose. (shared/attached for reference).
If they get social security, do they have to turn the funds back into SS or does the client get to name who they want to give the remaining funds in their account to? Any remaining funds in the client’s account at the time of death do not need to be returned to Social Security. but, any Social Security payment received after the date of death must be returned to the Social Security Administration. Can the client make a donation to the provider, has anyone had this situation? ILS: We do not accept donations from clients. GLI: A guardian once submitted a written letter to the provider expressing intent to make a donation. Bost: We do not receive monetary donations from clients. If a provider has a form like this, can they share it? Bost shared. (attached)
