Several providers expressed willingness to share Job Evaluation and Performance Evaluation forms through the DDPA peer network. Providers interested in obtaining samples were encouraged to reach out through the DDPA listserv.
Common elements found in IDD provider performance evaluation forms:
– Core competencies: communication, professionalism, adherence to client rights, safety practices
– Job-specific skills: documentation accuracy, medication administration (if applicable), behavioral support plan implementation
– Reliability: attendance, punctuality, responsiveness to schedule changes
– Training compliance: completion of required annual trainings
– Behavioral standards: interaction with clients, families, and coworkers
– Goal-setting section for the upcoming review period
Most providers reported conducting annual performance evaluations tied to wage increase decisions. Some providers use a 90-day review for new employees followed by annual reviews thereafter.
