DPSQA confirmed that for background check renewals on long-term employees, a new, current version of the background check application must be completed — the existing application on file from years ago is not acceptable for the renewal submission. The new application must include the attestation for the 5-year continuous Arkansas residency requirement that was added to the form.
Providers noted this creates additional administrative work for employees who have been with the agency for many years, particularly if information has changed (address, name, etc.). DPSQA acknowledged this concern and indicated that discrepancies in name or address between old and new applications should be documented with a brief explanation note rather than being a reason to delay the submission.
