2. Bookmark the DPSQA provider portal and check it periodically for updated forms before submitting applications.
3. When you receive correspondence from DHS that references a form, verify the form version matches the current version on the portal before using an older copy.
4. Contact your DDS regional office promptly when you discover a form version discrepancy — they can clarify which version is currently accepted.

DDPA continues to advocate for better advance notice from DHS when forms change and for revised forms to be included directly in regulatory communications.
