DHS/DPSQA clarified that the new background check application requirements apply to all new submissions going forward — including renewal (5-year recheck) submissions for existing employees. For renewals, you do not upload the employee’s old application; instead, a new, current background check application reflecting the employee’s current information should be completed and uploaded.

This means long-term employees who are due for their 5-year recheck will need to complete a fresh application. DPSQA acknowledged provider concerns about this burden and encouraged providers to contact the DPSQA provider helpline if they encounter specific issues during the renewal process. Check the DPSQA website for the most current version of the application form.
